PRIMARY BENEFITS:
If you checked the box for Benefit 1 and offer a Transit Benefit of $30 or more (or the full cost of a pass if it is less than $30), please enter the amount offered.
If you checked the box for Benefit 2 and offer a Vanpool Benefit of $30 or more (or the full cost of a pass if it is less than $30), please enter the amount offered.
If you checked the box for Benefit 4, please enter the estimated percentage of employee commute trips eliminated by telework.
If you checked the box for Benefit 5, please enter the estimated percentage of employee commute trips eliminated by compressed work week.
If you checked the box for Benefit 6 and offer at least $30 in lieu of Parking please enter the amount offered.
Primary Benefit 7: We offer an equivalent benefit that provides similar value to our employees, reduces traffic and air pollution and is agreed to by BWC. Please describe your equivalent benefit.
You are required to offer at least three (3) Supporting Benefits to employees (you need to provide only one (1) if you have fewer than 20 employees). Please check ALL that apply.
OPTIONAL: We also claim credit for one or more of the following LEED Alternative Transportation (Sustainable Sites) credits for our worksite as a supporting benefit.
